اجازه‌نامه پرداخت با کارت اعتباری

 

اینجانب، امضاءکننده‌ی زیر، به آژانس مسافرتی سفرهای زندگی و یا دیگر کنسولیترهای هوایی همکار و خطوط هوایی صادرکننده بلیت‌ها اجازه می‌دهم که هزینه پروازها یا سایر خدمات مسافرتی درخواستی مرا از کارت اعتباری ذکرشده در زیر دریافت نمایند. این مجوز برای یک یا چند تراکنش تا سقف مبلغ مشخص‌شده و برای مسافران ذکرشده که توسط من بازبینی و تأیید شده است، معتبر می‌باشد.

 

 پذیرش شرایط
من درک کرده و می‌پذیرم که این پرداخت‌ها ممکن است غیرقابل استرداد باشند و مشمول شرایط و ضوابط خطوط هوایی و آژانس مربوطه می‌شوند، از جمله محدودیت‌های مربوط به تغییرات، کنسلی‌ها و بازپرداخت‌ها که در وب‌سایت‌ها و قوانین بلیت درج شده است.

 

تأیید دارندۀ کارت
تأیید می‌کنم که کارت بانکی ارائه‌شده متعلق به من است یا برای این تراکنش مجاز به استفاده از آن می‌باشم. همچنین تأیید می‌کنم که هویت من برای این تراکنش از طریق تماس تلفنی تأیید شده و کد شناسایی خصوصی جهت فعال‌سازی ارسال این فرم را دریافت کرده‌ام. در صورتی که مسافر یا یکی از مسافران ذکرشده نباشم، همچنان یک نسخه از کارت شناسایی معتبر دولتی خود (با عکس) را جهت تأیید این مجوز ارائه خواهم کرد. این مجوز همراه با کارت شناسایی ارائه‌شده، مجوز کافی برای پردازش پرداخت توسط خطوط هوایی یا آژانس‌های همکار ما را فراهم می‌آورد.

برای اهداف تأیید، تنها وارد کردن ۴ رقم آخر کارت در این فرم نیاز است.

    1. Authorization
    I, the undersigned, authorize AB Travel Radar Ltd. (dba Zendegi Travels), its partnered air consolidators, and the airlines issuing the tickets to charge the credit card listed below for flights or other travel services I have requested. This authorization applies to one or more transactions up to the total amount indicated, for the passengers listed, which I have reviewed and approved.

     

    2. Acknowledgment of Terms
    I understand and accept that these charges may be non-refundable and are subject to the applicable airline and agency terms and conditions, including restrictions on changes, cancellations, and refunds, as stated on their websites and ticket rules.

     

    3. Cardholder Verification
    I confirm that the bank card provided is my own or that I am authorized to use it for this transaction. I also confirm that my identity for this transaction has been verified via phone, and I have received a Private Identification Code to activate the submission of this form. If I am not the traveler, or one of the travelers listed, I will still provide a copy of my government-issued photo ID to confirm this authorization. This authorization, together with the provided ID, grants sufficient permission for the airline or our partnered agencies to process payment using my card.

     

    4. Credit Card Information
    Only the last 4 digits of the card are required for this form for verification purposes.

    Full payment will be securely processed via our PCI-compliant payment gateway after authorization.
